Yes, I would use longer bolts for all spacers. For 12mm spacers you’ll need 12mm longer bolts. OEM bolt length is 30mm.

undoing the sway bar brackets and loosening exhaust hangers gave me enough play in the system to allow those bolts to slide out.

It has nothing to do with which gear we’re on. It’s just a certain frequency resonates more with the cable and makes it vibrates. It’s more noticeable if you’re under-geared and accelerating.

service manual says M6x20 and M6x40. The 2 center ones are 5mm allen and the ones around the cover are normal 8mm hex

Lotus must have changed the design. Mine has a hole where the buttons thread in. Threads are plastic and easily damaged, that’s prob why they switched to pins. Do the pins screw out?
